Why Lifestyle
Modern medicine has become so focused on drugs, scans and procedures that lifestyle intervention has been relegated to a footnote.
The data is clear: many of the cardiovascular conditions we treat have both a foundation and a solution in lifestyle intervention. Hypertension, metabolic disease, atrial fibrillation, elevated cholesterol — all respond profoundly to structured dietary and exercise change. Yet in conventional pathways, this is either absent or generic.
Patients are interested — and motivated. But those with medical conditions have had no trusted route to connect their diagnosis and treatment with equally rigorous dietary and exercise programmes. Cardiac rehabilitation in the NHS is offered only to a narrow subset of patients, and even then as a one-size-fits-all approach. Private options exist — nutritionists, personal trainers — but often feel disconnected from what is happening medically.
CardioFIT was built to close that gap: a medically led programme that brings exercise, dietary and medical expertise into one supervised pathway, with the same rigour we apply to procedural treatment. The programme has been evolved and informed by our own research — linking dietary and exercise intervention to modern cardiology diagnostics and treatments — supported by the NIHR, Heart Research UK, RUHX and the University of Bath.
The Programmes
Three routes in. One integrated approach.
Prehabilitation
Targeting recognised cardiovascular risk factors — hypertension, metabolic disease, early coronary artery disease — to reverse progression through lifestyle intervention before escalation to procedural treatment.
The Cardiovascular MOT →Rehabilitation
For patients with established cardiovascular conditions or events — following heart attack, angioplasty, or surgery — who are motivated to reduce the probability of recurrence.

Condition-specific programmes — most notably our AF Reversal Programme, where structured lifestyle intervention has a direct evidence base for rhythm improvement.

Full medical review
Every patient starts here. We ensure all medical elements and risks are considered before the programme begins.
Lifestyle & dietary review
You arrive with a week’s dietary diary. Dr Khavandi conducts the dietary review personally — the majority of the appointment — alongside goals and the roadmap. The report goes to your trainer team.
Your 10-week cycle
Twice-weekly one-to-one hour sessions with your CardioFIT-trained trainer, plus a weekly homework session where you reproduce the exercise protocol in your own time — three sessions per week in total. One session each week carries data documentation: body metrics, heart rate, blood pressure, and cardiovascular fitness. Formal fitness tests at start, midpoint and end.
Consultant review
Dr Khavandi reviews you at the end of the cycle, goes through the ten weeks of data with your trainer’s input, and builds the next cycle on it.

Each phase runs in 10-week, 20-session blocks. After consolidation, maintenance can flex to once per week — but the data loop and consultant oversight continue.
A Note on GLP-1 Medication
I was always sceptical of medical interventions for weight loss. But the GLP-1 agonists have been a genuine advance. We use them as an adjunct to the lifestyle programme — as a catalyst for patients who are overweight, or where progress has stagnated. We do not recycle patients through a programme that is not delivering results.
We prescribe directly — no need to navigate an online pharmacy. The prescription is issued electronically, you pay for the medication, and it ships to your home.
The Investment
CardioFIT is generally not covered by medical insurance — patients self-fund. We have structured the cost to be encouraging and to cover the resources involved. The cost tapers across the three phases: by maintenance, it aligns with standard personal training in Bath. The initial investment reflects the additional medical supervision.

The Professional Network
For personal trainers, nutritionists & allied professionals.
We have been working with a network of personal trainers and allied lifestyle professionals since 2016 — and we have learnt a great deal from each other. When we started, we found PTs who were motivated, capable, and actively seeking CPD to work with medical patients — but had no obvious route into the clinical world. Many held the exercise-referral accreditation, which I had never even heard of despite being a consultant deeply involved in exercise and lifestyle. That disconnect was the link we wanted to bridge.
We also work with nutritionists and practitioners in preventive care who do excellent work but often find themselves isolated from a medical pathway. Many undertake advanced biochemical screening — lipoprotein(a), for example — but when patients take those results to their GP, there is often nowhere for that information to go. We provide a way in: a link to medical diagnosis, treatment, and structured lifestyle intervention.
We are interested in hearing from gym facilities too — helping members connect medical care with lifestyle programmes and expanding the CardioFIT network.
On the horizon: a formal accreditation programme for trainers in the CardioFIT protocol, and a Professional Hub with case-based CPD modules. Both are in development.
